The three stage samples — both endpoints and the interior midpoint — combined in the weights (1, 4, 1) this rule derives from the authored endpoint quadrature. Those weights are Simpson's rule, and taking the interior sample at the Hermite interpolant's midpoint is what makes the whole scheme fourth-order.
